UPSC CMS 2022 Previous Year Questions

Paper I highlighted pharmacology of cardiac drugs, respiratory management protocols, and neonatology. Paper II featured trauma surgery, advanced laparoscopy concepts, complex obstetric scenarios, and national health indicators from NFHS-5. Gastroenterology continued its dominance from 2021 with 14 questions in Paper I, reinforcing that this subject had become a consistent high-yield area. Endocrinology matched Gastroenterology closely at 12 questions, with diabetes mellitus, thyroid disorders, and adrenal pathology featuring prominently. Haematology maintained a strong presence with 11 questions covering coagulation disorders, anaemia workup, and blood component therapy. In Paper II, Gynaecology led with 21 questions, followed by Obstetrics at 19, maintaining the OBG-heavy pattern established in earlier years. GI Surgery sustained its elevated share at 16 questions, confirming the structural increase first seen in 2021 was now a stable trend. PSM questions drew heavily from NFHS-5 data, testing knowledge of current national health indicators, a pattern that became more pronounced in subsequent years.

The 2022 answer key was published by UPSC with subsequent corrigenda. CMS Prep has incorporated all verified corrections in the question bank. A few questions in the Pharmacology and Surgery sections had answer changes in the corrigenda, making it essential to use the final revised key rather than the initial provisional release. The 2022 paper also had a higher-than-usual number of disputed questions in the PSM section, where national health programme data had multiple valid interpretations.

Gastroenterology continued its upward trajectory, reaching 14 questions. Hepatitis, pancreatitis, and GI bleeding management were the most frequently tested sub-topics. Endocrinology at 12 questions included complex diabetes management scenarios and adrenal insufficiency. Toxicology saw a notable increase to 7 questions (up from 4 in 2020), covering drug interactions, heavy metal poisoning, and occupational toxins. Pulmonology dropped to 6 questions, and Neonatology also declined, suggesting a redistribution of Pediatrics weightage toward older age-group paediatrics.

GI Surgery held strong at 16 questions, covering appendicitis, intestinal obstruction, biliary surgery, and basics of minimally invasive techniques. General Surgery increased to 11 questions with a focus on hernias, thyroid surgery, and breast conditions. Environmental Health appeared as a distinct tested topic, reflecting UPSC's interest in public health determinants. Neurosurgery at 3 questions tested head injury classification and basic intracranial pressure management. Surgical Oncology dipped to 3 questions, lower than its 2020 levels.

2022 maintained the multi-statement trend from 2021 and pushed harder into Gastroenterology and Endocrinology clinical scenarios in Paper I. GI Surgery case-based questions became more prominent in Paper II. The paper consolidated the pattern shift seen in 2021, confirming that UPSC had moved away from pure recall toward applied clinical reasoning. Questions required candidates to connect pharmacological mechanisms with clinical decisions, and surgical questions increasingly presented patient scenarios requiring step-by-step management rather than single-fact answers. The difficulty was rated as moderate-to-high, with the Toxicology increase adding an element of surprise for candidates who had deprioritized the subject.

The 2022 paper is where the modern UPSC CMS pattern truly stabilized. While 2021 introduced multi-statement and clinical formats, 2022 confirmed these were permanent features, not one-time experiments. For preparation purposes, this paper is an excellent benchmark for the difficulty level you should target. The Gastroenterology and GI Surgery dominance established here continued through 2023, and the NFHS-5 data integration in PSM signaled UPSC's expectation that candidates stay current with national health surveys.
