UPSC CMS Posting & Departments 2026
UPSC CMS recruits for four departments under the Central Government. Understanding where you will be posted, your promotion path, and PG opportunities is essential before deciding to appear for the exam.
CHS is the largest and most sought-after posting under UPSC CMS. CHS doctors work as General Duty Medical Officers (GDMOs) in Central Government hospitals including CGHS dispensaries, ESI hospitals, port health offices, airport health offices, and border health posts.

Postings: All India posting, transferable. You may be posted anywhere from Andaman & Nicobar Islands to Ladakh. Seniority and preference determine transfer requests.
PG opportunity: CHS doctors can appear for NEET PG in-service quota with 10% additional marks. In-service candidates have a reserved quota in government medical colleges.
Railway Medical Officers are posted across zonal railway hospitals, divisional hospitals, and health units. Posting is within your allocated railway zone (e.g., Northern Railway, Southern Railway). This limits all-India transfers compared to CHS.
Key features:
- Zonal posting — less likely to be transferred across India
- Free rail travel for self and family (pass entitlement based on grade)
- Railway residential quarters available at most postings
- In-service PG available through Railway in-service NEET quota
- Promotion path: Junior Scale → Senior Scale → Junior Administrative Grade → SAG
Both NDMC and MCD postings are within Delhi. If you want to stay in Delhi and have family commitments, these are attractive. However, vacancies are fewer and the posting environment is primarily OPD and community health work in Delhi’s municipal areas.
NDMC: Covers Lutyens Delhi area. Better infrastructure. Fewer doctors, so workload per doctor may be higher.
MCD: Covers outer Delhi areas. Larger organisation, more hospitals and dispensaries.
